Method

The remainder of this chapter describes the evaluation of a pilot project of Community Assessment and Rehabilitation Teams in Cornwall. This was a joint project between the Cornwall Healthcare Trust, Cornwall and Isles of Scilly District Health and Family Health Service Authorities, Cornwall County Council Social Services Department, and Tor and South West College of Health Studies. It was funded through joint funding in the year 1993–94 and the evaluation was completed during 1994–95.

The first and main aim of the CARTs project was to examine how the community assessment and rehabilitation needs of disabled and elderly people could best be met. It was anticipated that this should be through better deployment of existing resources, ensuring a fit with the care management process in the production of care packages, and augmented with selective development of targeted rehabilitation resources in the community.

A second aim of this study was to supply both purchaser and provider management organisations with periodic information to feed into their business plans to allow for more effective provision of health care, to complement that provided by the social care agencies, private sector providers, and primary care teams in support of community care.

The referral criteria were as follows:

• Patients who have a major need for rehabilitation in order to help them remain in their own homes.

• Patients who have a particular problem with daily living activities which need to be addressed at home.

• Patients for whom early discharge can be facilitated by community rehabilitation.

• Patients who require multidisciplinary assessment prior to entry to a nursing home.

• Patients who require interventions to prevent hospital admission, including nursing assessment of complex needs, respite care and carer support.
